The NDIS supports Australian’s under 65 with a permanent disability that impacts their everyday life.

If you think you might be eligible for the NDIS, you can download an Access Request Form (ARF) and a Supporting Evidence Form (SEF), work with your GP, allied health, and specialist medical professionals to prepare and provide the information required.
