Do I need to fill out forms? 

Yes, there are certain details we need about you ahead of your assessment date. We have made this very easy and our referrals co-ordinator will walk you through the process. Links to these forms will be provided in your welcome email.

What do I wear? 

We ask our clients to wear loose, comfortable clothing and shoes suitable for standing or walking in, as appropriate for you.

Who shall I bring with me? 

Anyone from your support network is welcome to attend our clinic with you.
